1. The opinions to be expressed in court including the basis and reasons.

2. Data and information considered while forming the opinions.

3. Exhibit to be used as a summary or support for the opinions.

4. Your qualification including a list of your recent publications.

5. The compensation to be paid to you after your job.

6. List of other cases in which you have testified in recent years.
