Best Form Builder Software for Mac PC

I am using MAC OS X. Is there a good form builder especially made for MAC? If so, what’s the name of the software and where can I download it for free. Please give a rating 1 up to 10. Ten is the highest. Thanks a lot. Hope you could answer my need to have a good form builder MAC.

There are numerous form builder available online, and the good news is some of them are free like JotForm but with limited and simple functions that can do. If you need more complex and easy to use software i suggest you go to CoffeeCup Web Form Builder but this software would cost you $69.
