I’ve got two individual networks and subnetworks that are connected with each other via CAT5 cable. What do I need to set up so that I can connect the host of the two networks to other host outside my network?

You will need a router to connect that network you already have to other networks. What the router will basically do is route the packets from that network to the networks outside. Since you have said that you already have the network set in your place, then I am assuming that you already have acquired the following:
-
A switch to connect all the computers in the network.

A patch panel to provide a termination point for the cables that will be running to all computers and other devices like printers too.

A server to store the information on the network. A server is just a computer only that it should have very large storage space and memory for it will be holding all the information on the network.
You will need to note that you will have a router on both your network and the network you want to join outside. If they are more than two networks that you want to join, then you will need to have a router for each of the network.
