How to create a simple sample of leave form template electronically?

Hi David,

It would look like the below image:

You can create an excel file with this information and save this in a shared network or internet server where every staff has access to. They can simply go there put the information’s and print and submit the application to the HR. Hope that this will be helpful for you.
