How will I check mbr online?

I want to apply to children's medical security plan (CMSP) under Medical benefit request (MBR).

I do not know the process of applying online.

Can you please explain me with step by step procedure how will I check mbr online?

How will I check mbr online?

Hi Nelson,

In order to apply for CMSP, you need o fill out the MBR form. You can get the form:

• By downloading the form from the MassHealth web site: http://www.mass.gov/eohhs/consumer/insurance/masshealth-apply/applications-and-member-forms.html
• By calling the MassHealth Enrollment Center at 1-800-531-2229 (TTY:1-888-665-9997)
• By visiting a local community health center (CHC).

In case of query, you can call CMSP Customer Service at 1-800-909-2677.

 

After the form is filled completely, sign and date it and send it to

 

MassHealth Enrollment Center

Central Processing Unit

P.O. Box 290794

Charlestown, MA 02129-0214

 

Make sure you answer all the questions in the application form. If there are any questions that do not apply, enter N/A. The information you give about you and your family should be correct and complete.
