I need a php contact form builder that will let me customize and to have at least CAPTCHA or something that would prevent spam. Please give me some recommendations for php contact form builder.

Hello Dorothy. PHP formMail Generator is what you are looking for. It has unlimited standard form fields, saves submitted form data as Excel files and prevents spam (CAPTCHA) among others things. You can get a copy here:
