Verizon modem fails to connect due to user name

Hi there,

This error appeared on my desktop and I tried to connect to the Internet using my Verizon modem. The user name and the password was inserted correct and I don’t know why this error appeared. Can anyone tell me more about this error ? How can I fix this one ?
