I'm just wondering if you formatted your PC and was not able to temporarily save your files to another storage for back-up purposes while formatting the PC because every time you format a drive, all the files that exist will be lost. Â
That's why when you try to recover those files on the drive, the files are already unrecoverable.
However, certain viruses or Trojans could also cause unrecoverable errors.

I’m quite confused. It’s not clear if this problem happens when opening files or an error suddenly appears out of nowhere telling of an unrecoverable error on the hard drive. Any of these reasons is true, you surely need to backup your data more often so you will not lose data in case of a device failure.
If the unrecoverable format error appears when opening a file or files, it means the file you are opening may probably be corrupted. And if your file is already in this state, I don’t think there is still a solution on fixing it unless you have a backup copy of the file then you can restore it or use the file instead.
This problem can also happen when copying files to an external drive like for example in a USB flash drive and then untimely removing the drive without even finishing the copying of the files. This will result on corrupting the file affected since the whole file is not complete.
This is the same with downloading files which are not yet complete. Disconnecting your computer from the internet while still downloading a file will surely corrupt the file. If an error of this sort appears on your computer, it is best to check your hard drive for errors using a disk utility application.
